456.013 (7), Florida Statute
The boards, or the department when there is no board, shall require the completion of a 2-hour course relating to prevention of medical errors as part of the licensure and renewal process. The 2-hour course shall count towards the total number of continuing education hours required for the profession. The course shall be approved by the board or department, as appropriate, and shall include a study of root-cause analysis, error reduction and prevention, and patient safety. In addition, the course approved by the Board of Medicine and the Board of Osteopathic Medicine shall include information relating to the five most misdiagnosed conditions during the previous biennium, as determined by the board. If the course is being offered by a facility licensed pursuant to chapter 395 for its employees, the board may approve up to 1 hour of the 2-hour course to be specifically related to error reduction and prevention methods used in that facility.

64B3-11.001 Continuing Education.
(1) In order to renew a clinical laboratory personnel license, a minimum of 24 contact hours of continuing education shall be earned during each biennium including a minimum of one contact hour for each of the categories in which the individual is licensed, and one contact hour of continuing education on HIV/AIDS. Also, as a part of the 24 continuing education hours, each licensee shall take a Board approved 2-hour course relating to the prevention of medical errors, which shall include root-cause analysis, error reduction and prevention, and patient safety. Directors and supervisors are required to obtain one contact hour of continuing education in administration and supervision. As part of the minimum of 24 contact hours of continuing education, each licensee shall be required to take a one hour course on Florida laws and rules governing clinical laboratory personnel or attend a public meeting of the full Board at which disciplinary actions are addressed. A telephone conference call meeting of the Board will not satisfy this requirement.

(4) Each Hearing Aid Specialist shall attend and certify attending a Board-approved two-hour continuing education course relating to the prevention of medical errors.  Licensees who are licensed under other professions regulated by Chapter 456, F.S., shall only be required to take one medical errors course to satisfy the continuing education requirements for this Board.  The two-hour course shall count toward the total number of continuing education hours required for license renewal.

64B8-13.005 Continuing Education for Biennial Renewal.

(c) Completion of two hours of continuing medical education relating to prevention of medical errors which includes a study of root cause analysis, error reduction and prevention, and patient safety, and which is approved by any state or federal government agency, or nationally affiliated professional association, or any provider of Category I or II American Medical Association Continuing Medical Education. One hour of a two hour course which is provided by a facility licensed pursuant to Chapter 395, F.S., for its employees may be used to partially meet this requirement. The course must include information relating to the five most mis-diagnosed conditions during the previous biennium, as determined by the Board. While wrong site/wrong procedure surgery continues to be the most common basis for quality of care violations, the following areas have been determined as the five most mis-diagnosed conditions: cancer; neurological conditions; acute abdomen related conditions; timely diagnosis of surgical complications; diagnosis of pregnancy related conditions.

64B9-5.011 Continuing Education on Prevention of Medical Errors.

(1) All licensees must complete a two hour course on prevention of medical errors, which meets the criteria of Section 456.013, F.S., as part of the total hours of continuing education required for initial licensure and biennial renewal.

(2) To receive Board approval, each course on prevention of medical errors shall consist of a minimum of at least two (2) hours of classroom or an equivalent home study program and shall include at a minimum the following subject areas:

(a) Factors that impact the occurrence of medical errors,

(b) Recognizing error-prone situations,

(c) Processes to improve patient outcomes,

(d) Responsibilities for reporting,

(e) Safety needs of special populations,

64B14-5.005 Mandatory Courses.

(1) Mandatory courses consist of the following:

(b) Two hours relating to the prevention of medical errors, which shall include a study of root cause analysis, error reduction and prevention, and patient safety; and

(3) Mandatory courses may be used for multiple registrations, examinations or initial license applications and for license renewal, if they are used within two years of the date the courses were completed.

(4) Mandatory courses set forth in paragraphs (1)(a), (b) and (c) must be individually approved by the Board.

64B15-13.001 Continuing Education for Biennial Renewal.

(1)(a) Every person licensed pursuant to Chapter 459, F.S., except those licensed as physician assistants pursuant to Section 459.022, F.S., shall be required to complete forty (40) hours of continuing medical education courses approved by the Board in the twenty-four (24) months preceding each biennial renewal period as established by the Department. Five of the continuing medical education hours for renewal shall include a one hour Risk Management Course, one hour Florida Laws and Rules, one hour on the laws regarding the use and abuses of controlled substances, and two hours Prevention of Medical Errors Course. Beginning in the 2010 - 2012 licensure biennium, five of the continuing medical hours for renewal shall include one hour of professional and medical ethics education, one hour Florida Laws and Rules, one hour on the federal and state laws related to the prescribing of controlled substances, and a two hour Prevention of Medical Errors Course.

(3)(a) For purposes of this rule, risk management means the identification, investigation, analysis, and evaluation of risks and the selection of the most advantageous method of correcting, reducing, or eliminating identifiable risks and domestic violence as defined in Section 741.30, F.S.

(3)(b) The continuing medical education found in paragraph 64B15-13.001(1)(a), F.A.C., with regard to Risk Management, Florida Laws and Rules, controlled substances and the prevention of medical errors shall be obtained by the completion of live, participatory attendance courses. However, the continuing medical education found in paragraph 64B15-13.001(1)(c) and (d), F.A.C., with regard to HIV/AIDS and domestic violence, may be obtained by the completion of non-live/participatory attendance.

(3)(d) 4 A licensee may use no more than five (5) hours of continuing education in the area of risk management for the purpose of completing the continuing education requirements for each biennial renewal.

(3)(f) For purposes of this rule, a two hour Prevention of Medical Errors course shall include a study of root cause analysis, error reduction and prevention, and patient safety. The course shall address medication errors, surgical errors, diagnostic inaccuracies, and system failures, and shall provide recommendations for creating safety systems in health care organizations. The course must include information relating to the five most mis-diagnosed conditions during the previous biennium, as determined by the Board. The following areas have been determined as the five most mis-diagnosed conditions: cancer, wrong-site/patient surgery, acute abdomen, heart conditions, and surgical complications/errors.

Note:  Pharmacists and Registered Pharmacy Technicians are required to complete Medication Errors, not Medical Errors.

(2) Requirements for Renewal.

64B17-8.002 Requirements for Prevention of Medical Errors Education..

(1) To receive Board approval for biennial renewal, courses on medical error prevention shall be two contact hours and include a study of root-cause analysis, error reduction and prevention, and patient safety, which shall encompass:
(a) Medical documentation and communication;
(b) Contraindications and indications for physical therapy management; and
(c) Pharmacological components of physical therapy and patient management.
(2) Applicants for initial licensure must have completed at least two contact hours of medical error education The Board shall accept coursework from accredited schools of physical therapy provided such coursework was completed after January 1,2002.
(3) If the course is being offered by a facility licensed pursuant to Chapter 395, F.S., the Board may apply up to one hour of the two-hour course if specifically related to error reduction and prevention methods used in that facility.
(4) The course may be used as part of the home study continuing education hours.
(5) Medical errors education courses approved by any Board within the Division of Medical Quality Assurance of the Department of Health pursuant to Section 456.003, F.S., are approved by this Board.

64B18-17.005 Continuing Education Requirements After Initial Licensure.

During the first biennium or within twelve (12) months of initial licensure, whichever ends later, practitioners are required to obtain five (5) hours of continuing education in the subject area of risk management by attending one full day of a meeting of the Board of Podiatric Medicine at which disciplinary hearings are conducted. The practitioner is then exempt from any other continuing education requirements for his or her first renewal except for hours mandated for prevention of medical errors and HIV/AIDS.

64B19-13.003 Continuing Psychological Education Credit.

(1) Continuing psychological education credit will be granted for:

(f) Attainment of diplomate status in a specialty area from the American Board of Professional Psychology, for which thirty-seven (37) continuing psychological education credits, not including the two-hour continuing education course on domestic violence required by Section 456.031(1), F.S., and the two-hour continuing education course on the prevention of medical errors required by Section 456.013(7), F.S., will be allowed only during the biennium during which the diplomate is first awarded;

(3) As a condition of biennial licensure renewal, each licensee must complete forty (40) hours of continuing psychological education.

(b) Two (2) of the forty (40) hours must relate to prevention of medical errors. In addition to the study of root-cause analysis, error reduction and prevention, and patient safety, the course content shall also be designed to discuss potential errors within a psychological setting, such as inadequate assessment of suicide risk, failure to comply with mandatory abuse reporting laws, and failure to detect medical conditions presenting as a psychological disorder. If the course is offered by a facility licensed pursuant to Chapter 395, F.S., for its employees, the Board will approve up to one (1) hour of the two (2) hour course to be specifically related to error reduction and prevention methods used in that facility.

(c) Passage of the laws and rules examination of the Board constitutes forty (40) hours of continuing education credit, including credit for professional ethics and Florida Statutes and rules affecting the practice of psychology. Passage of the laws and rules examination, however, does not satisfy the requirement for the two (2) credit hours of continuing education on domestic violence required every third biennial licensure renewal period, nor the requirement for two (2) hours relating to prevention of medical errors.

(4) Each applicant and all licensees shall take a Board approved 2-hour continuing education course relating to the prevention of medical errors. The 2-hour course shall count toward the total number of continuing education hours required for each license renewal. The course shall include medication errors, including missed treatments, documentation errors, equipment errors, patient errors and communication errors.

Every six years, each licensee shall complete two hours of continuing psychological education on domestic violence as defined in Section 741.28, F.S., as a part of and not in addition to the hours required for biennial licensure renewal. The licensee shall retain for six years certificates of attendance or other records to document the completion of the continuing education requirement. The Deparment will audit at random a number of licensees as is necessary to assure that the continuing education requirements are met.

(3) Effective January 1, 2002, all applicants for initial or renewal of initial license or licensure by endorsement shall submit to the Board proof of completion of a two (2) hour continuing education course relating to the prevention of medical errors. The 2-hour course shall count toward the total number of continuing education hours required for the profession. The course shall be provided by a Board-approved continuing education provider and shall include a study of root-cause analysis, error reduction and prevention, and patient safety.
